Chronic Fatigue


A 56-year-old man presented complaining of chronic fatigue. Physical examination was normal except for some pallor of the nail beds. History elicited complaints of a sharp, burning epigastric pain after eating which was relieved by fasting. Some relief was obtained by OTC antacids. Lab studies showed a hypochromic, microcytic anemia. Iron therapy was begun. Barium swallow studies showed a defect in the mucosa of the duodenum. Below is an image of duodenum from a similar case.

 

What further test should be done?

1. Gastrin levels
2. Test for presence of H. pylori in stomach
3. Endoscopy to look for a carcinoma of the duodenum
4. 1 and 2
5. 2 and 3
