Proteus is a urease-producing bacterium which renders the urine alkaline with a pH of 7 or greater favoring the precipitation of struvite.  These stones can quickly become large enough to block urine outflow and cause hydronephrosis.   The hydronephrosis does not lead to hypertension until the cortex, and thus the glomeruli, are severely affected.  In the case shown, the cortex is virtually absent.  The kidney would not normally be removed for this condition; the infection would be treated as would the hypertension.
